Describe the obstacle precisely

After a session, ask whether participants understood the goal, could follow the instruction and could complete the supported task. These are different questions. Confusing terminology may need a better explanation; a broken resource needs repair. A task outside the available product scope needs an honest boundary, not another tutorial.

Assign the appropriate response

Group observations by instruction, content, access, product behaviour or organisational decision. Give each group an owner and a next action. Do not count a session as successful simply because the facilitator found a manual workaround. The proposed review should make it possible to improve learning materials while also surfacing problems that customers cannot solve through additional effort.

Imagine this

Hypothetical programme scene

Hypothetically, several learners cannot open the same approved sample document. Reviewing the session shows they understood the instructions. The owner repairs the resource and retests it, rather than scheduling another lesson that would repeat the same failure.

Make it practical.
